Who We Are
Community Care Connections of Wisconsin (CCCW) is certified by the State of Wisconsin to operate as a Managed Care Organization in seventeen (17) counties in north central and northwest Wisconsin. Counties served by CCCW include: Ashland, Barron, Bayfield, Burnett, Douglas, Iron, Langlade, Lincoln, Marathon, Polk, Portage, Price, Rock, Rusk, Sawyer, Washburn, and Wood. Our administrative office is located in Stevens Point – with 14 other office locations established across the remainder of our service region.
Community Care Connections of Wisconsin is formed as a Long Term Care District and operates under Wisconsin State Statutory language created in 2007. It is governed by an (11) eleven-member Board of Directors representing the interests of residents residing in the counties served.
Membership in Community Care Connections of Wisconsin is determined through a functional/fiscal eligibility determination and enrollment process conducted by the seven (7) local Aging & Disability Resource Centers (ADRC) serving our service region. Elders and adults with physical and/or developmental disabilities may be eligible for Family Care participation. Participation in the Family Care program is voluntary.
CCCW provides each participant, or member, with an Interdisciplinary Service Coordination Team that supports the development of a Member Centered Care Plan. This Care Plan is focused on identifying and supporting individual outcomes that are important to the member; is based on a comprehensive assessment process;, and is developed through the authorization of services, supports, and goods available through the Family Care benefit package.
Over 30 different services and goods are included in the Family Care benefit package, which are delivered through a state certified network of over 1,400 CCCW contracted providers.
