Welcome. Thank you for visiting the website of Community Care Connections of Wisconsin (CCCW). My name is Mark Hilliker and I have the privilege of serving as CCCW’s Chief Executive Officer.

Community Care Connections of Wisconsin is one of eight Managed Care Organizations in Wisconsin that supports the lives of elders and adults with intellectual or physical disabilities through the State of Wisconsin’s Family Care program. Family Care provides long-term care supports and services to eligible seniors and adults with disabilities. By the end of 2015, Family Care will support people in 64 of Wisconsin’s 72 counties.

CCCW is certified by the State of Wisconsin to operate as a Managed Care Organization in seventeen (17) counties in north central and northwest Wisconsin. Counties served by CCCW include: Ashland, Barron, Bayfield, Burnett, Douglas, Iron, Langlade, Lincoln, Marathon, Polk, Portage, Price, Rock, Rusk, Sawyer, Washburn, and Wood. Our administrative office is located in Stevens Point – with 14 other office locations established across the remainder of our service region.

Our organization began operations on July 1, 2008 and currently serves over 6,000 individuals in meeting their personal outcomes on a daily basis. CCCW employs over 400 exceptional staff members and contracts with over 1,400 high quality service providers to support the needs of those served by the organization.

As an organization, we work diligently to enhance the experiences of those supported by the Family Care program. In 2013, CCCW trademarked an innovative managed care delivery model, called Commonunity®, which focuses on individuals living meaningful lives as full citizens in their communities. Commonunity® is a strength-based approach to delivering managed care support and it focuses attention on five key component areas:

  • Support for Self-determination and self-direction
  • Enhanced Community Living options
  • Increased opportunities for Integrated employment
  • Expanded Community connections and
  • Improved mobility.

We are very proud of the fact that CCCW has been recognized for the past three years as having the highest member satisfaction among all Managed Care Organizations statewide, based on Member Satisfaction Survey results compiled by the State of Wisconsin. At the same time, we have been able to lower our organizational costs to a point where we meet all State required fiscal solvency and reserve requirements.

The Board of Directors and staff at CCCW are committed to providing high-quality, individualized, and cost-effective supports and services to Family Care members and their families. We're also committed to working very closely and collaboratively with service providers and other community organizations in order to provide the very highest levels of long-term care support across north central and northwest Wisconsin.
